Primary Care Case Management (PCCM) Overview

PCCM is a primary care provider network where providers contract directly with HHSC. PCCM clients receive health care services through a primary care provider. PCCM enlists primary care providers to provide a medical home, where clients receive the majority of their primary care and obtain referrals for specialty care. For providing a medical home to enrolled clients, participating primary care providers are paid traditional fee-for-service rates plus an additional $5.00 per patient, per month for those who selected them as their primary care provider. Providers who do not wish to be a primary care provider are not required to sign a primary care provider agreement.

PCCM is a network of Medicaid primary care providers and hospitals—not a health maintenance organization (HMO). PCCM improves the provision of primary and preventive care services to Medicaid clients while ensuring the efficient use of limited Medicaid resources.

Who Can Be a Primary Care Provider

PCCM enlists primary care providers to provide a medical home, where clients receive the majority of their primary care and obtain referrals to specialty care. Although clients are required to receive services and referrals through their primary care provider, referrals may be made to any Medicaid-enrolled provider. In addition, primary care providers are responsible for hospital admissions and obtaining prior authorization for elective inpatient and outpatient procedures. OB/GYN, Texas Health Steps (THSteps), vision services, and behavioral health providers may continue to see Medicaid clients without a primary care provider referral. However, coordination with the patient's primary care provider is encouraged. A primary care provider can be a:

  • Physician (general practice, family practice, internal medicine)
  • Physician Assistant
  • Pediatrician
  • Obstetrician/Gynecologist (OB/GYN)
  • Federally qualified health center (FQHC)
  • Rural health clinic (RHC)
  • Advanced practice nurse (i.e., nurse practitioner, certified nurse midwife)
  • Specialist who would like to serve as a primary care provider for patients with special needs (e.g., oncologist, pulmonologist)

      FQHC and RHC Providers Claims For PCCM Clients Require Referring Provider Effective April 1, 2010
      Information posted February 5, 2010: Effective April 1, 2010, Federally Qualified Health Center (FQHC) and rural health clinic (RHC) providers must submit the referring provider’s National Provider Identifier (NPI) on claims for outpatient hospital or professional services that are provided to a PCCM client for whom they are not the primary care provider. If the referring provider’s NPI is not submitted on a claim for outpatient hospital or professional services that were provided to a PCCM client for whom they were not the primary care provider, the claim will be denied.       PCCM Primary Care Providers Medical Records Are Routinely Audited by TMHP
      Information posted February 5, 2010: Reminder: Medical records are subject to routine audits by Primary Care Case Management (PCCM) staff to ensure quality and continuity of care.       Clarification to “Updates to Services Provided to PCCM Clients by FQHCs and RHCs” Effective 3/1/2010
      Information posted January 22, 2010: This is a clarification to an article that was published on this website on January 6, 2010, titled “Update to Services Provided to PCCM Clients by FQHCs and RHCs.” The article listed “Freedom of Choice Services”. These services are called freedom of choice because Medicaid Managed Care clients may self-refer for some services. The following is the updated and correct information. Effective March 1, 2010, FQHC and RHC providers that provide gynecological services to PCCM clients must submit claims with an encounter code and modifier GY in the claim details.       Reminder About Submitting Provider Complaints
      Information posted August 25, 2009: Providers have the right to submit a complaint to the TMHP Complaints Resolution Department to express any dissatisfaction with Primary Care Case Management (PCCM).
